Amanecer’s Intensive Programs offer a full range of compassionate, culturally competent mental health services to children, transition-age youth (TAY), ages 18-26, and Adults from 26-60+. Each client is assigned a treatment team that may include a therapist, case manager, psychiatrist/nurse practitioner, and substance use counselor, as appropriate. Team composition is based on the individual’s needs and eligibility criteria for the specific program. We provide quality mental health services by ensuring our clients are enrolled in a program that will provide the appropriate level of care for our client’s current needs. Services are provided via telehealth, in the office, in clients’ homes, or in the community to meet clients’ treatment needs and goals.
Our intensive case management services may provide access to resources that support vocational training, employment, education, housing, and other essential services that promote independence and overall well-being. Our psychiatric team is made of bilingual/bicultural psychiatrists and nurse practitioners that are to provide mediation support services.
Full-Service Partnership (FSP-ICM)
Full Service Partnership (FSP-ICM) is designed to serve severe adults experiencing moderate to significant functional impairments who may not meet criteria for a higher level of care but continue to experience serious behavioral health challenges and would benefit from intensive community-based support.
FSP-ICM provides a comprehensive array of services through a coordinated, multidisciplinary, whole-person approach. The program is designed to support individuals in improving daily functioning, incresaing housing stability, enhancing employment and educational outcomes, reducing involvement with the criminal justice system, and minimizing reliance on emergency medical and psychiatric services, including emergency room visits and hospitalizations.
Child and Youth Full Service Partnership – High Fidelity Wraparound (FSP-HFW)
Child and Youth Full Service Partnership-High Fidelity Wraparound (FSP-HFW) is an evidence-based enhancement of Intensive Care Coordination (ICC) for children, adolescents, and transitional-age youth. The model positions the Child and Family Team (CFT)-including the youth, family, natural supports, and service providers-as the central driver of care planning and service delivery.
Children and youth enrolled in FSP-HFW often experience complex and co-occuring challenges, including serious behavioral health conditions, trauma, substance use concerns, housing instability, and involvement or risk of involvement with multiple systems such as child welfare and progration.
Services are delivered through a coordinated, strengths-based, family-driven, and team-centered process. The program is commited to providing individualized clinical and non-clinical supports that help children, youth, and families make meaningful progress toward their goals. FSP-HFW is designed to increase access to behavioral health services, improve school engagement and functioning, reduce untreated behavioral health needs, and decrease the likelihood of intututionalization, justice-system involvement, and out-of-home placement.
